	First hour prepaid @ $80.00, plus all applicable telephone charges.
	Discussions going beyond the first hour are charged at $20.00 per 15 minute increment or portion thereof.
	Appointment time will be set up and confirmed by letter or email.

	$25.00 per additional 15-minute increment or portion thereof, plus facility rental.
	Travel time allowance – ½ hour one-way. Additional travel time billed at $7.50 per 15 minute increment or portion thereof.
